Trust Us For Life

What Is LifeLinx?

LifeLinx is a new free comprehensive approach to hypertension management that may be available to you at no cost.

It uses a wireless blood pressure cuff and readings that are captured by an Android tablet.

Those results are automatically reviewed by LifeLinx personnel and simultaneously sent to a team of clinicians and hypertension specialists.

If your readings are concerning, clinical professionals use the data to address your specific health issues and prescribe exact medications to help you control your blood pressure!

About our Partner Upward Health

Upward Health is a national, risk-bearing multidisciplinary medical group delivering 24/7 in-home primary, behavioral health, urgent, and post-discharge care, contracting with health plans across product lines in value-based arrangements to manage complex, chronically ill patients.

Why do we need your MBI number?

We ask for your MBI for two reasons:
1. To confirm your ORIGINAL MEDICARE insurance plan for eligibility (sorry, Advantage members are not eligible).
2. Remote medical personnel used for your hypertension care are health professionals and they need the same official information your neighborhood doctor requires in order to provide services.
